Calcitriol for Cats and Dogs with Kidney Disease User Group - Dr. Larry Nagode, the leading expert on calcitriol use in CRF cats and dogs, is a frequent participant in discussions here.  The group also has excellent "bookmarks" and "files" sections with many links to valuable documents concerning calcitriol.  If you're using or are considering using calcitriol to treat your CRF cat, membership in this group is a must!

Michigan State University (MSU) College of Veterinary Medicine - Veterinary Diagnosis Laboratory does a Parathyroid Hormone and Ionized Calcium test (Order Code 20033).  The price for your vet is $42.  Per reports from members of the Calcitriol Support Group, price is about $70-$90 per for the test, including your local vet's costs for shipping and handling and markup.  Antech Labs also does this test but at a higher cost.
